New: Clinical Reasoning Assessment Now Available for Paramedic

We're excited to announce that Clinical Reasoning Assessment (CRA) is now available for the Paramedic provider level in EMSTesting — at no additional cost.


The format of the new CRA was inspired by the National Registry's Clinical Judgment exam and consists of two parts: students first complete an adaptive test, then move into a scenario-based section. In the scenario section, students work through a single patient encounter across the En-Route, On-Scene, and Post-Scene phases of a call, using the same interactive technology-enhanced item (TEI) question formats (multiple choice, multiple response, drag-and-drop, build list, and options box) used in EMSTesting and at the NREMT. This structure helps students apply their knowledge to realistic, evolving patient encounters, building skills that matter both in the field and on exam day.


Once activated, CRA has its own dedicated experience for both students and instructors:

  • Students will see a new "Clinical Reasoning" button under the class listing to access their test.

  • Instructors will see a new "CRA Results" button on their homepage under the class listing. Results are shown on a single report across six domains — the first five reflect adaptive test performance, and the sixth reflects clinical reasoning (scenario) performance.


Where to find it: In your class, go to Edit Class, then Adaptive, and you'll see the new Clinical Reasoning Assessment option to activate it.


Best practices for getting started:


  • Introduce CRA after students have completed most of their didactic instruction, so scenarios reinforce what they've already learned.

  • Use it during National Registry preparation to build familiarity with Clinical Judgment scenario questions before their certification exam.

  • Encourage students to read each phase of the scenario carefully before answering — that's where the real reasoning practice happens.

  • Revisit CRA scenarios more than once over the course of a program to reinforce growth over time.
